Insurance Coverage & Costs

Participating Insurances

Reproductive Medicine Associates of Northern California is in network with most major insurance plans that offer coverage for fertility benefits, and we offer financial coordinators who will work with you to confirm your benefits before your first visit. We’ll help you navigate the complexities of the process and provide basic guidelines that will help you understand your options.

In network with:

  • Aetna
  • Anthem Blue Cross
  • Blue Shield of California
  • Cigna
  • United Healthcare
  • Progyny
  • Future Family

If we do not participate in an individual’s specific health plan, the individual may still be eligible for coverage for a percentage of the costs of certain treatments as an out-of-network benefit through their own insurance plan. Patients with nonparticipating insurance companies will be required to provide payment upfront for all services. Payment will be expected at the time of service for all non-IVF services and two weeks prior to the start of the IVF cycle.

Using the above example and applying to a typical IVF cycle, the following is an approximate cost:

Cost: $ 10,010.00*
Deductible: $250.00
Coinsurance: $1,793.00

The estimated insurance reimbursement to you for cycle = $7,265.00

* Estimated IVF Cycle cost does not include ICSI, Assisted Hatching, or Medications.

** Remember that your insurance company will reimburse you using their reasonable and customary rates. For example, if your insurance pays 80% for out of network services and we charge $50 but the insurance r&c rate is $40, they will reimburse you at 80% of the $40 amount, not our full fee.

Prices may be subject to change without notice.

If you want more information about infertility benefits from your insurance company, consider asking whether the below are covered:

  • Diagnostic testing
  • Imaging related to ovarian stimulation for in vitro fertilization (IVF) or egg freezing
  • In Vitro Fertilization (IVF) — how many cycles?
  • Intrauterine insemination (IUI) — how many cycles?
  • Fertility medications—if yes, is there a total monetary medication “allowance,” a maximum number of vials, or other product limitations?
  • Are there any limitations to my benefits, such as age, medical conditions, or prior cycles?
  • Can you please provide a written explanation of my benefits?
